2. What this policy covers

This website, telephone contact with the office, and the personal health information collected in the course of your care.

3. This website

No forms

This website has no forms, so it cannot receive messages. Nothing you do on it sends the practice your name, your contact details or any health information. To reach the office, please telephone or write, using the details above.

Cookies, analytics and tracking

This website sets no cookies. It runs no analytics or visitor-tracking scripts and no advertising pixels. Its fonts, images and scripts all come from this website itself, so reading these pages sends information about you to no company other than the website’s host, Cloudflare (see Hosting below). The one exception is the map on the contact page, described below.

What your browser keeps

To make return visits faster, and to show a simple page if your connection drops, your browser may keep copies of this website's own pages, images and files on your device. While a tab is open, the site may also remember where you were on a page, so that going back or reloading returns you to the same place, and whether you have already seen the home page's opening animation. All of this stays in your browser, is never sent to the practice or to anyone else, and can be removed at any time by clearing this site's data in your browser's settings.

Hosting

This website is hosted on Cloudflare Pages. As with any website, Cloudflare's servers receive your device's IP address and the technical details of each request, such as the page asked for, the time and the type of browser, in order to deliver the page. Cloudflare handles this information under its own privacy policy. It is not linked to any clinical record.

The map on the contact page

The contact page shows a map of the office supplied by Google Maps. It is not loaded with the rest of the page: your browser asks Google for it only when you reach the map, which on a screen tall enough to show it straight away means as soon as the page opens. From then on Google receives your device's IP address, may set its own cookies, and serves the map's images and fonts from its own servers, all under Google's privacy policy. The office address is also written beside the map.

Links to other websites

The Instagram section on the home page shows copies of the practice's own posts, stored on this website; your browser does not contact Instagram unless you follow a link to it. Those links, the "Get directions" link to Google Maps, the link to Dr. Khetani's entry on the public register of the College of Physicians and Surgeons of Ontario, and links to other organisations all lead to websites the practice does not control, each with its own privacy policy. When you follow one, the other website is told only that you came from this website, not which page you were reading.

4. Telephone calls and email

Calls to the office line, (905) 639-2244, are answered by Phelix, an AI telephone assistant built for medical offices and provided to the practice by an outside company. It takes your details and the reason for your call and passes them to the office team, so that someone can act on them. Faxes to the office, such as referrals from physicians, are received through the same service. You can ask to speak with a person, and if you would rather not give health details to the assistant, you can keep them for the office team or for your appointment.

This website does not publish an email address. Ordinary email and text messages are not secure, so please do not send health information that way; telephone the office, or raise it at your appointment.

5. Your health information

What is collected, and why

When you are referred to or treated by the practice, it collects personal health information: your identity and contact details, your health card number, the referral and the reason for it, your history, examination findings, images from nasal endoscopy and imaging, clinical photographs, test results, treatment plans, operative and follow-up notes, and correspondence with the other members of your care team. It comes from you, and from your referring physician or nurse practitioner and the hospitals and laboratories involved in your care. It is used and shared to assess and treat you, to keep the other clinicians involved in your care informed, to arrange surgery, to bill the Ontario Health Insurance Plan or you for services, and to meet the practice's legal and professional duties.

Consent

Within your circle of care (your referring physician or nurse practitioner, and the hospitals, laboratories and imaging departments involved in your care) Ontario law allows the practice to rely on your implied consent. You can limit or withdraw that consent by telling the office, subject to the exceptions PHIPA sets out. Sharing your information with anyone outside your circle of care needs your express consent, unless the law requires or permits it.

Clinical photographs

Photographs taken for surgical planning are part of your record and are stored securely with it. They are never published, shown to other patients, or used in teaching without your separate written consent, which is asked for as its own decision, never as a condition of care, and which you may withdraw at any time.

Research

The practice takes part in clinical research. Taking part in a study is always voluntary and never a condition of care.

When the law requires disclosure

In limited circumstances the practice must disclose information without consent: where a law requires it (for example mandatory reporting duties), to the College of Physicians and Surgeons of Ontario in the course of its regulatory work, or in response to a court order or warrant. Only the information required is disclosed.

How it is protected

PHIPA requires the practice to take reasonable steps to protect your information against theft, loss and unauthorised use or disclosure, and to keep and dispose of records securely. If a privacy breach affects you, you will be told as PHIPA requires, and the Information and Privacy Commissioner is told where the law requires it.

6. Your rights

You have the right to ask what information the practice holds about you, to see and have a copy of your record, and to ask for it to be corrected if something in it is inaccurate or incomplete. Make the request in writing to the office, at the address above. The practice must respond within 30 days, or tell you within that time if it needs up to 30 days more, and must explain any part of a request it cannot meet. A fee may be charged for copies; if so, you will be told the amount before it is charged.

7. Raising a concern or making a complaint

Please raise any concern with the office first, by telephone at (905) 639-2244 or in writing to the address above. You also have the right to complain to the Information and Privacy Commissioner of Ontario, which oversees PHIPA: www.ipc.on.ca (opens in a new tab) · 1-800-387-0073 · 2 Bloor Street East, Suite 1400, Toronto, Ontario M4W 1A8.

8. Changes to this policy

When this policy changes, the new version is published on this page with its effective date shown at the top.
